Meadow Falls of Conneaut is seeking a compassionate Licensed Practical Nurse (LPN) to join our long-term care team. Offers 12-hour shifts, every other weekend, and a $2,500 sign-on bonus, with hourly pay between $28 and $30.

Qualifications

• Current LPN license.
• Experience as an LPN.

Responsibilities

• Provide direct patient care, including administering medications, performing treatments, and assisting with daily living activities.
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team.
• Monitor and report changes in patient condition to the nursing staff and medical team.
• Collaborate with other healthcare professionals to ensure comprehensive care and support.
• Document patient information accurately and thoroughly in accordance with facility policies and procedures.
• Participate in ongoing education and training to maintain current knowledge and skills.
• Demonstrate a commitment to providing excellent customer service and promoting a positive and respectful work environment.

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Meadow Falls of Conneaut, a premier long-term care facility dedicated to providing exceptional care and services to our residents. As an LPN at Meadow Falls of Conneaut, you will have the opportunity to make a meaningful difference in the lives of our residents and their families. Every other weekend and 12 hr shifts.
