Qualifications

• Active, unrestricted LPN license in New Jersey.
• Experience in long-term care or skilled nursing preferred but not required.
• Strong clinical assessment and communication skills.
• Ability to work effectively in a fast-paced healthcare environment.

Responsibilities

• Deliver high-quality nursing care to residents per individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor resident conditions and promptly report changes to the care team.
• Document resident care using PCC electronic health records.
• Collaborate with RNs, CNAs, physicians, therapists and other healthcare professionals.
• Assist with admissions, discharges, and resident assessments.
• Ensure compliance with federal, state, local, and facility regulations.
• Promote resident dignity, comfort, safety, and quality of life.

Licensed Practical Nurse (LPN) Responsibilities

As a Licensed Practical Nurse (LPN) in our skilled nursing facility, you will:

• Deliver high-quality nursing care to residents in accordance with individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor resident conditions and promptly report changes to the interdisciplinary care team
• Accurately document resident care using PointClickCare (PCC) electronic health record software
• Collaborate with Registered Nurses, Certified Nursing Assistants, physicians, therapists, and other healthcare professionals
• Assist with admissions, discharges, and resident assessments
• Ensure compliance with all federal, state, local, and facility regulations
• Promote resident dignity, comfort, safety, and quality of life

Qualifications

• Active, unrestricted Licensed Practical Nurse (LPN) license in the State of New Jersey
• Long-term care, rehabilitation, subacute, or skilled nursing experience preferred but not required
• Experience with PointClickCare (PCC) is a plus
• Strong clinical assessment and communication skills
• Ability to work effectively in a fast-paced healthcare environment
