Job summary

Yonkers Center for Nursing and Rehabilitation is seeking Licensed Practical Nurses (LPN) to join our skilled nursing team in Yonkers, NY. Various shifts are available including day, evening, and night, with a $10,500 sign-on bonus and competitive pay.

Full-time, Part-time, and PRN options are offered, with comprehensive benefits and weekly payroll. Duties include admissions, medication administration, and coordination with CNAs and the interdisciplinary team to ensure high-quality resident care.

Qualifications

• LPN licensed by the State
• CPR Certification required
• Two Years prior nursing experience preferred

Responsibilities

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• May supervise Certified Nursing Assistant (CNA)
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment setup, maintenance and use.
