Job summary

Hudson Hill Center for Rehabilitation and Nursing in Yonkers, NY is seeking a dedicated LPN to join our care team and support rehabilitation and long-term residents.

Shifts available 7am-3pm, 3pm-11pm and 11pm-7am. Responsibilities include admissions, medication management, and supervising CNAs under supervision while following policy and infection control guidelines.

Qualifications

• LPN license valid in NY state.
• CPR certification required.
• Two years nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• May supervise Certified Nursing Assistant (CNA).
• Follow infection control, emergencies, and center policies.
• Collaborate with interdisciplinary team for optimal outcomes.
• Report abnormal changes in residents' condition per policy.
• Deliver nursing care for long-term or rehabilitative patients.
• Document data and observations.
• Implement and evaluate patient plans of care.
• Direct and supervise care in selected situations.
• Maintain required documentation.

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents

• Medication requisitions with pharmacy and physicians

• Medication administration

• May be responsible to supervise Certified Nursing Assistant (CNA)

• Following infection control, emergencies, and other center policies

• Collaborating with resident’s interdisciplinary team to provide the best outcomes

• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy

• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.

• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.

• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.

• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.

• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.

• Maintains knowledge of necessary documentation requirements.

• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.

• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State

• CPR Certification required

• Two Years prior nursing experience preferred
