The Addictions Care Center of Albany in Albany, NY is seeking an LPN or RN to support medical staff in stabilization and rehabilitation services for residential clients.

You will screen medical needs, monitor vitals, administer medications, document in MAR and 10e11, and participate in weekly case conferences, all while maintaining confidentiality and safety.

Qualifications

  • Active NY LPN or RN license.
  • CPR and First Aid certification.
  • Two years nursing experience preferred, residential setting preferred.

Responsibilities

  • Participates in screening of clients' medical needs and admission information.
  • Works with medical staff to meet treatment needs of each client.
  • Provides group counseling sessions on health-related issues.
  • Observes medications are properly taken by clients.
  • Documents medications in MAR and 10e11.
  • Take vitals of clients as ordered by doctor.
  • Gather and submit urine drug screens to labs.
  • Document lab results in client 10e11 charts promptly.
  • Monitors medical needs and reports findings to medical provider.
  • Ensures confidentiality and safety of all residential clients.
  • Logs and communicates accurately at shift changes.
  • Prepares reports/records as program procedures require.
  • Participates in weekly case conferences as needed.
  • Assists in incident reporting involving client medical occurrences.
  • Make calls to local ambulance, fire department, police, or CDPC as needed.
  • Attends required staff meetings and training.
  • Performs other duties as assigned and works in other residential buildings as needed.

Job description

Primary Purpose

Assists medical staff in providing health care needs for medically supervised stabilization and rehabilitation services.

Responsibilities
  1. Participates in screening of clients' medical needs, gathering necessary admission information/data, and observation of physical, mental, and emotional status of residential clients.
  2. Works cooperatively with managerial and medical staff to meet the treatment needs of each client
  3. Provides group counseling sessions to clients regarding health-related issues and concerns
  4. Observe that all medications are properly taken by clients
  5. Document medications taken in MAR and 10e11.
  6. Take vitals of clients as ordered by doctor.
  7. Gather and submit all urine drug screens to the labs
  8. Document in client 10e11 charts all lab results in a timely fashion
  9. Monitor clients for medical needs and report findings to medical provider
  10. Ensures confidentiality and safety/security of all residential clients.
  11. Logs and verbally receives/delivers accurate information at change of shifts.
  12. Prepares necessary reports/records as required by program procedure.
  13. Participates in weekly case conferences as needed and/or directed by supervisor
  14. Assumes responsibility reporting incidents involving client medical occurrences.
  15. Make calls to local ambulance service, fire department, police, or CDPC, as needed.
  16. Attends required staff meetings and training.
  17. Performs other duties as assigned and works in other residential buildings as needed.
Education Requirements

Accredited L.P.N. or R.N. program.

License Requirements

Candidate must have NYS LPN or R.N. license, and NYS Driver's License. Must be CPR and First Aid certified.

Experience Requirements

Two (2) years nursing experience preferred, preferably in a NYS OASAS residential facility.

Shift is Monday - Friday 7am-3pm
