Job summary

West Chester Rehabilitation & Healthcare Center is hiring an LPN for full-time, part-time, or PRN shifts (7a–7p or 7p–7a). Daily Pay is available. The role involves delivering safe, person-centered nursing care under an interdisciplinary team in a resident-focused setting.

Required PA LPN license and CPR/BLS certification, with strong clinical judgment and communication skills to support residents’ physical and emotional needs.

Qualifications

• Active LPN license in PA.
• CPR/BLS certification required.
• Ability to manage multiple patients in a fast-paced setting.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow individualized care plans with Nurse Manager and team.
• Administer medications and treatments as prescribed.
• Monitor residents and report changes promptly to supervisor.
• Maintain accurate documentation per policies and standards.

$30/hr-$39/hr (all inclusive)

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of PA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
