Job summary

National HealthCare Corporation (NHC) is seeking an LPN for NHC HomeCare Low Country in Charleston, SC. Under RN supervision, you will provide skilled nursing care per the plan of care and document patient teaching and responses.

You will instruct patients and caregivers on disease management, observe care outcomes, and suggest plan adjustments to the RN/Clinical Manager. Home health experience is preferred.

Qualifications

• Unencumbered LPN license in the applicable state or compact state
• Graduate of a state approved LPN program
• Minimum one year clinical experience as an LPN
• Candidates with less than one year experience will be mentored and provided oversight through the end of that year
• Experience in home health care is desirable

Responsibilities

• Under the supervision of a Registered Nurse, provides skilled nursing services as ordered on the plan of care and according to policy/procedures and state practice acts.
• Instructs the patient/caregiver in various aspects of patient care and disease management, and documents comprehension and compliance with teaching.
• Observes patient response to care/teaching and makes recommendations regarding revisions to the plan of care to the Registered Nurse.
• Provides and documents skilled nursing care according to the established plan of care, policies/procedures, and standards of care; performs other duties as assigned by the RN/Clinical Manager.

Licensure

• Unencumbered, current Licensed Practical Nurse license in the state where the HomeCare agency is located or compact state if applicable
• Graduate of a state approved LPN program
• Minimum one (1) year clinical experience as an LPN
• Individuals with less than one full year’s experience as an LPN will be considered. If hired, they will be mentored and provided additional oversight through the end of that 1-year period.
