National HealthCare Corporation (NHC) HomeCare Midlands is seeking an LPN to join our Midlands team. The LPN provides skilled nursing services under RN supervision and delivers patient education as part of the care plan.

The role emphasizes accurate documentation, care plan adherence, and timely communication with the RN and clinical manager. Ideal candidates have an active LPN license, at least one year of clinical experience, and a willingness to be mentored during the first year.

Qualifications

• Current practical nurse license in the relevant state or compact state.

Responsibilities

• Under RN supervision, provide skilled nursing services as ordered on the plan of care.
• Instruct patient/caregiver in care and disease management and document understanding and compliance.
• Observe patient response and revise the plan as needed with the RN.
• Provide and document skilled nursing care according to the plan of care and policies.

LPN Position Highlights

• Under the supervision of a Registered Nurse, provides skilled nursing services as ordered on the plan of care and according to policy/procedures and state practice acts.
• Instructs the patient/caregiver in various aspects of patient care and disease management. Documents the patient/caregiver comprehension of and compliance with teaching.
• Observes patient response to care/teaching and makes recommendations regarding revision of plan for patient care to the Registered Nurse.
• Provides and documents skilled nursing care according to the established plan of care, policies/procedures, and standards of care. Performs other duties as assigned by the RN / Clinical Manager.
