The Licensed Practical Nurse (LPN) provides skilled nursing services to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings throughout Pennsylvania. Working under the supervision of a Registered Nurse (RN) Case Manager, the LPN implements individualized care plans, administers medications, performs clinical procedures, and supports individuals and families in maintaining health, safety, dignity, and quality of life.
This position operates within the Nursing Department and reports directly to the RN Case Manager or Director of Nursing. The LPN delivers hands-on clinical care consistent with Pennsylvania regulations and OneWell Health Care standards. Services are provided in participant homes, community settings, and other approved service environments.
The Licensed Practical Nurse (LPN) is responsible for implementing individualized care plans; administering prescribed medications and treatments; performing clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring; and supporting individuals with chronic condition management such as diabetes, epilepsy, and respiratory conditions. The LPN monitors and documents physical, emotional, and behavioral changes; promptly reports changes in condition to the RN Case Manager; and maintains accurate, timely documentation in compliance with regulatory and organizational standards. This role provides education and training to families, caregivers, and new nursing staff; participates in interdisciplinary meetings and case reviews; maintains infection control and safety practices in all care environments; supports quality improvement initiatives; assists with internal audits and clinical compliance reviews; participates in required training and professional development; and performs additional duties as assigned within the scope of licensure to ensure coordinated, compliant, and person-centered care delivery.
The LPN is responsible for safeguarding all Protected Health Information (PHI) and confidential participant information in accordance with HIPAA, state law, and OneWell Health Care policies. Unauthorized disclosure of confidential information is strictly prohibited.
Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
Ability to communicate effectively in English, both verbally and in writing. Additional languages are considered an asset based on participant population needs.
Valid driver’s license in good standing and reliable transportation required. Must meet OneWell Health Care motor vehicle eligibility requirements.
Local travel within assigned Pennsylvania service regions is required. Travel may include participant homes, community settings, and occasional attendance at meetings or trainings.
