Qualifications

• Active Maine nursing license required.
• Up‑to‑date vaccinations (MMR, varicella, Hep B, seasonal flu).
• Ability to work every other weekend with on‑call responsibilities.
• Experience in assisted living is a plus.

Job Summary

Pinnacle Health & Rehab at Canton is seeking a dynamic and highly motivated Licensed Practical Nurse (LPN) to join our clinical team. As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care in a 66-bed Assisted Living facility. Responsibilities

• Provide direct patient care under the supervision of a Registered Nurse (RN) or other licensed healthcare providers
• Lead and oversee a clinical team, working collaboratively with direct care workers and ancillary departments
• Develop and implement individualized care plans for residents, ensuring a high level of customer satisfaction
• Monitor and report changes in residents' conditions to the RN or other licensed healthcare providers
• Participate in quality improvement initiatives and maintain accurate and up-to-date records
• Collaborate with other healthcare professionals to ensure seamless continuity of care

Requirements

• Active Maine nursing license
• Proof of MMR, varicella, Hep B, and seasonal flu vaccination (Oct-Apr)
• Ability to work every other weekend with on-call responsibilities
