Job summary

Community Options, Inc. in Parsippany, NJ is hiring a Licensed Practical Nurse to deliver care in a community setting. The L.P.N. will follow the nursing process and interact with individuals, families, and health team members, maintaining professional nursing standards.

Starting pay is $30.00 per hour. The role requires a nursing license, one year of relevant experience with disabilities, and a valid driver’s license. Employment is contingent on background checks and drug testing.

Qualifications

• Bachelor’s degree or equivalent experience in nursing field.
• Minimum of one year of experience providing nursing services to individuals with disabilities.
• Valid nursing license.
• Valid driver’s license with a satisfactory driving record.
• Ability to calculate figures related to medication dosages.
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
• Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
• Advise supervisor of all concerns and issues related to the provision of nursing services
• Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions of individuals and refer for medical services as needed
• Monitor the use of medications through medication administration logs
• Ensure program documentation and billable records are completed accurately and timely
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
• Additional tasks and responsibilities may be assigned
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 11 states. We are now hiring a skilled Licensed Practical Nurse in Parsippany, NJ. The L.P.N. delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.
