Community Options, Inc. is hiring a Licensed Practical Nurse to provide community-based nursing care in Mechanicsburg, PA. The LPN will use the nursing process—assessment, planning, intervention, and evaluation—to support individuals with disabilities and collaborate with health teams.

Starting pay is up to $36.00 per hour. The role requires a valid nursing license, a college degree or equivalent experience, and driving capability for community-based visits. Background checks are required.

• Valid driver’s license with a satisfactory driving record.
• Ability to calculate figures related to medication dosages.
• Ability to establish a comfortable and supportive relationship with an individual receiving supports.
• Ability to effectively present facts and recommendations.
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing.

Responsabilidades

• Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law.
• Ensure individuals attend medical appointments per their ISP.
• Advise supervisor of all concerns and issues related to the provision of nursing services.
• Develop and update ISP/PCP with an interdisciplinary team to include concerns, observations, and behavioral developments.
• Train employees on the medical needs of assigned individuals.
• Monitor health and medical conditions of individuals and refer for medical services as needed.
• Monitor the use of medications through medication administration logs.
• Ensure program documentation and billable records are completed accurately and timely.
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families.
• Additional tasks and responsibilities may be assigned.
• Cooperate with license and department staff in inspections, inquiries, or investigations.

We are now hiring a skilled Licensed Practical Nurse in Mechanicsburg, PA.

The L.P.N. delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.
