Brockton Post Acute Care in Brockton, MA is seeking a Licensed Practical Nurse (LPN) to provide compassionate, patient-centered care in a sub-acute and long-term care setting.

You will follow individualized care plans, administer medications, monitor residents, and document outcomes. We offer full-time and part-time roles, daily pay, shift differentials, and a supportive team that values growth and work-life balance.

Qualifications

• Current, unencumbered active LPN license in MA.
• BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with accepted standards of nursing practice and the physician's plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per the plan of care and facility policies.
• Monitor residents’ conditions, report changes to the supervising nurse or physician, and respond to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.
