Job summary

Cape Cod Post Acute is seeking an LPN to join our dedicated nursing team on a full-time night shift (11pm–7am). This position offers daily pay, a $12,000 sign-on bonus, and a competitive hourly rate ($37–$46) with shift differentials.

We value compassionate, patient-centered care and provide a supportive environment with opportunities for professional growth within a 135-bed sub-acute and long-term care facility. MA LPN license and BLS required.

Qualifications

• Current, unencumbered LPN license in MA (active).
• BLS certification (CPR) required.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow individualized care plans created by Nurse Manager and team.
• Administer medications and treatments per POC and facility policy.
• Monitor residents and report changes promptly to supervising staff.
• Maintain accurate documentation per orders and regulatory standards.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of MA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
