Job summary

Port Washington Post Acute is seeking Licensed Practical Nurses (LPNs) to join our care team in a busy post-acute setting. You will provide direct nursing care, supervise CNAs, and coordinate with RNs to ensure residents receive high-quality care.

We welcome new grads and experienced nurses alike. The role offers full-time and part-time options with various shifts, and a supportive leadership team focused on resident well-being and professional development.

Qualifications

• Licensed Practical Nurse (LPN) license current or eligible in Washington state.
• Graduate of an accredited nursing program.
• Valid IV certification required.
• Strong communication and assessment skills required.
• Compassionate, dependable and committed to excellent resident care.
• Experience in SNF or post-acute care preferred but new grads welcome.

Responsibilities

• Provide nursing care, administer medications, and complete treatments per physician orders.
• Monitor residents' conditions and report changes promptly to RNs and physicians.
• Collaborate with interdisciplinary team to develop and update care plans.
• Supervise CNAs and assist with staff development and training.
• Maintain accurate documentation in compliance with facility policies and regulatory standards.
• Promote resident dignity, comfort and quality of life.

Position Summary

We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our care team. As an LPN, you will provide direct nursing care to residents, supervise CNAs, and collaborate with RNs and other team members to ensure residents receive the highest quality of care.
