Job summary

Nuclear Care Partners in Augusta, GA seeks a Licensed Practical Nurse (LPN) to deliver in-home, one-on-one skilled nursing care in patients' homes. You will work with a mission-driven, nurse-led team that values patient dignity and outcomes.

Responsibilities include providing skilled care, developing and following individualized care plans, accurate documentation, and collaborating with interdisciplinary teams while maintaining compliance with state regulations.

Qualifications

• Current LPN license in GA.
• Current CPR certification.
• Valid driver's license and reliable, insured vehicle.
• Home health experience preferred but not required.
• Must pass pre-employment screenings.

Compensation

Compensation is based on experience, shift type, and market factors. The listed range reflects entry-level to highly experienced applicants.
