Job summary

Signature HealthCARE is seeking an LPN/LVN to provide direct nursing care in a skilled nursing facility, including medication administration and wound care, while supervising nursing staff to ensure patient safety and quality care.

The role requires strong clinical skills, familiarity with EHR, and the ability to coordinate admissions and care plans within a collaborative interdisciplinary team.

Qualifications

• Current, active LPN/LVN license in state of practice.
• CPR/BLS certification preferred.
• Minimum one year clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with EHR systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the assigned shift in accordance with policies.
• Monitor and observe residents’ status; notify physician of changes.
• Develop, implement, and review individualized resident care plans with the team.
• Complete accurate and timely clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing or designee.

How you Will make a Difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.

• Effective verbal and written communication skills.
