Altera Health and Rehabilitation Center in Fairhope, AL is seeking compassionate LPNs to join our team in a skilled nursing and rehabilitation facility. You will provide direct resident care, administer medications, and support the clinical team in delivering high-quality care.

Requirements include an Alabama LPN license, experience in long-term care or rehab preferred, strong clinical skills, and the ability to work weekends/holidays as needed.

Qualifications

Responsibilities

• Provide direct nursing care according to individualized care plans.
• Administer medications and treatments as ordered.
• Monitor resident condition including vital signs and changes in status.
• Complete accurate and timely documentation.
• Assist with admissions, transfers, and discharges.
• Work closely with RNs, CNAs, physicians, and therapy staff.
• Support residents with daily care needs when needed.
• Participate in care planning and family meetings.
• Follow infection control and safety procedures.
• Help train and support new nursing staff.

Altera Health and Rehabilitation Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our growing team in Fairhope, Alabama.In this role, you will provide direct resident care, administer medications, and support our clinical team in delivering high-quality care in a skilled nursing and rehabilitation facility.

LPN Requirements

• Active Licensed Practical Nurse (LPN) license in Alabama
• Experience in long-term care, skilled nursing, or rehab preferred
