Job summary

Genesis HealthCare is seeking a full-time Licensed Practical Nurse (LPN) for our Agawam, MA assisted living community. The LPN will work under the supervision of an RN to deliver nursing care, support resident well-being, and maintain accurate documentation.

The ideal candidate has a valid LPN license, BLS/CPR certification, and the ability to communicate effectively in English. Schedule includes MWF 3P-10P and an on-call weekend once a month.

Qualifications

• Graduate of an approved school for practical nursing or vocational nursing.
• Current Licensed Practical Nurse license by the State Board of Nursing.
• English language proficiency to ensure patient safety and care.

Responsibilities

• Collect, report, and document patient data and changes in condition to the RN.
• Contribute to individualized care plans with goals and interventions.
• Administer medications and perform treatments as ordered by physicians.
• Communicate pertinent patient data to the RN and/or physician.
• Supervise patient care by unlicensed staff per policies and procedures.
• Utilize PCC for documentation and maintain accurate drug counts.

As an LPN, you'll work under the direction of a Registered Nurse (RN) to deliver efficient and effective nursing care, ensuring positive clinical outcomes and resident satisfaction.

Position Highlights

• Collect, report, and document patient data, observe and report changes in condition to the RN.
• Contribute to individualized patient care plans, including establishing goals and developing interventions.
• Administer medications and perform treatments as ordered by physicians.
• Communicate pertinent patient data to the RN and/or physician.
• Monitor and supervise patient care provided by unlicensed staff, ensuring job duties are performed according to policies and procedures.
• Utilize Point Click Care (PCC) for documentation and maintain accurate drug counts.

Qualifications

• Must be a graduate of an approved school for practical or vocational nursing.
• Current Licensed Practical Nurse license by the State Board of Nursing in the center’s state is required.
• Must be able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our patients to respond to their medical and physical needs.
• Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy.
• Maintain current BLS/CPR certification (as required by State).

Posted Salary Range

USD $31.00 - USD $36.00 /Hr.
