Job summary

Genesis HealthCare in New Hampshire is seeking a part-time LPN to join our Assisted Living team on the 11 PM–7 AM shift. You will provide compassionate nursing care under the direction of an RN, focusing on resident well-being and independence.

The role involves collecting patient data, updating care plans, administering medications, and coordinating with the RN and physicians to ensure safe, quality care. Nursing license and BLS/CPR are required.

Qualifications

• Graduate of an approved school for practical or vocational nursing.
• Current LPN license as required by the state's board of nursing.
• Must be able to read, write, speak, and understand English.
• Maintain current BLS/CPR certification.

Responsibilities

• Collect, report, and document patient data; observe changes and report to RN.
• Contribute to individualized patient care plans with goals and interventions.
• Administer medications and perform treatments as ordered by physicians.
• Communicate pertinent patient data to the RN and/or physician.
• Monitor and supervise care by unlicensed staff.
• Utilize PCC for documentation and maintain accurate drug counts.

As an LPN, you'll work under the direction of a Registered Nurse (RN) to deliver efficient and effective nursing care, ensuring positive clinical outcomes and resident satisfaction.

Position Highlights

*Collect, report, and document patient data, observe and report changes in condition to the RN.

*Contribute to individualized patient care plans, including establishing goals and developing interventions.

*Administer medications and perform treatments as ordered by physicians.

*Communicate pertinent patient data to the RN and/or physician.

*Monitor and supervise patient care provided by unlicensed staff, ensuring job duties are performed according to policies and procedures.

*Utilize Point Click Care (PCC) for documentation and maintain accurate drug counts.

Qualifications

*Must be a graduate of an approved school for practical or vocational nursing.

*Current Licensed Practical Nurse license by the State Board of Nursing in the center’s state is required.

• Must be able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our patients to respond to their medical and physical needs.

• Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy.

*Maintain current BLS/CPR certification (as required by State).

Posted Salary Range

USD $29.00 - USD $33.50 /Hr.
