Job summary

STGi is seeking a Licensed Practical Nurse/Licensed Vocational Nurse to provide comprehensive nursing care at a Community Based Outpatient Clinic. The role supports patient check-in/out, medication administration, and coordination with the PACT team.

The nurse will perform procedures, observe responses to treatment, maintain documentation in VISTA, and assist in scheduling and patient education while ensuring confidentiality and safety for veterans.

Qualifications

• Performs nursing procedures including injections, wound care, catheterization and venipunctures.
• Maintains infection control practices and universal precautions as part of daily tasks.
• Documents patient information in medical records and the VISTA system to ensure continuity of care.
• Schedules initial and follow-up appointments in line with VA access standards and medical necessity.
• Provides patient education materials and supports privacy and safety for patients and families.

Job description

STGi is currently seeking a Licensed Practical Nurse/Licensed Vocational Nurse to provide services at our Community Based Outpatient Clinic.

JOB SUMMARY:

The general duty of the Licensed Practical Nurse/Licensed Vocational Nurse is to provide a wide variety of clinical and administrative nursing care under the direction of the CBOC Clinic Manager, PACT Provider and STGi Program Manager.

ESSENTIAL FUNCTIONS:

• Demonstrates basic knowledge of Pharmacology and medication administration techniques per facility protocols, and state regulations, as prescribed by PACT Provider.
• Works with the PACT team to check-in/check-out patients.
• Participates in all staff meetings and PACT huddles.
• Observes patients and reports adverse reactions to medications, treatment or physical/mental condition.
• Performs cardiopulmonary resuscitation (CPR) and assists during respiratory and cardiac arrest procedures.
• Performs a range of nursing procedures, including dressing changes, suture removal, oxygen administration, wound irrigation, catheterization, injection administration and venipunctures.
• Prepares patients for laboratory procedures and examinations by providing information and instructions, while assisting the Primary Care Provider and RN Care Manager upon request.
• Sets up and operates therapeutic and emergency equipment, including monitor, EKG, oxygen, automatic electronic defibrillator (AED), and portable suction.
• Maintains universal precautions and infection control practices.
• Assists support staff as needed.
• Documents all pertinent patient information in their medical records and VISTA to demonstrate quality care delivery and promote continuity of care.
• Assists in maintaining the VISTA (Veteran Health Information Systems and Technology Architecture) clinic reminder tracking system current for each enrolled patient.
• Documents all pertinent information and completes the VISTA Clinical Reminder Program as outlined by the site-specific VAMC for compliance of the Directors Performance Measures.
• Assists in scheduling patients for initial and follow-up appointments in accordance to the VA access standards and the patient's medical necessity.
• Assures patients are informed regarding their plan of care, including lab and diagnostic test results as needed.
• Provides health educational materials and resources to patients and their families for informational purposes.
• Participates in the ongoing Performance Improvement Program between STGi, its Subcontractor(s), and the VAMC.
• Assists in ensuring that all required reports are completed accurately.
• Assists the CBOC Clinic Manager and other staff in maintaining the clinic’s compliance with all federal, state, local, JCAHO, OSHA, Veteran Affairs, STG VA Program and Subcontractor safety and operational regulations, directives and standards.
• Assists the RN Care Manager in ensuring all patient information on access/wait time documentation is complete and accurate.
• Maintains confidentiality of all information and supports patients' privacy, rights, and safety.
• Applies the basic concepts of customer service techniques, cultural diversity and age-specific characteristics.
• Documents patient reminders, clinical reminders, clinical interaction and closes encounters, answering questions, and performs other duties as needed to ensure a beneficial visit, online activity and/ or training and support of the PACT team.
• Makes follow up visits.
• Provides general clerical/administrative support coordination work for the unit.
• Performs other work-related duties as assigned.
