Job summary

Avamere Skilled Advisors, LLC in Keizer, Oregon is seeking a Licensed Practical Nurse (LPN) for day and evening shifts. This position can be full-time or part-time at Avamere Court at Keizer.

The LPN provides basic nursing care under RN supervision, including vital signs, medication administration, wound care, hygiene, mobility, and assistance with daily activities.

You will collaborate with CNAs and other staff to ensure high-quality resident care and safety in a long-term care setting.

Qualifications

  • Must hold an LPN license in good standing.
  • Active CPR/BLS certification during employment.
  • Experience in long-term care or geriatric settings preferred.
  • Experience with EMR documentation.

Responsibilities

  • Administer medications as prescribed under RN supervision.
  • Take and record vital signs and monitor residents.
  • Perform wound care and assist with daily activities.
  • Document care, vitals, and observations accurately.
  • Maintain infection control and resident safety.

Licensed Practical Nurse LPN

Job Summary

Licensed Practical Nurse provides basic nursing care, including vital signs, medication administration, wound care, and assisting residents with daily activities, under the supervision of a Registered Nurse (RN)

Essential Duties and Job Responsibilities

Basic Nursing Care:

  • Vital Signs:Accurately measure and record vital signs like temperature, pulse, blood pressure, and respirations.
  • Medication Administration:Administer medications as prescribed by a physician, following facility protocols and under the supervision of an RN.
  • Wound Care:Perform wound care, including changing dressings and monitoring for signs of infection.
  • Hygiene and Mobility:Assist residents with personal hygiene, dressing, bathing, and mobility.
  • Nutrition and Hydration:Ensure residents receive proper nutrition and hydration, assisting with feeding if necessary.

Patient Monitoring:

  • Patient History:Record a patient's history and current condition.
  • Observation:Observe and document changes in a resident's condition, reporting any concerns to the RN.
  • Communication:Communicate effectively with residents, families, and other healthcare professionals.

Collaboration and Supervision:

  • RN Supervision:Work under the supervision of an RN, following their guidance and instructions.
  • CNA Supervision:Supervise certified nursing assistants (CNAs) in providing basic nursing care.
  • Teamwork:Collaborate with other healthcare professionals to ensure residents receive comprehensive care.
  • Other Duties:
  • Patient Safety:Maintain a safe environment for residents, following infection control protocols and other safety guidelines.
  • Documentation:Accurately document all care provided, including medications administered, vital signs, and any observations.
  • Emotional Support:Provide emotional support and comfort to residents and their families.
Requirements and Qualifications
  • Must possess a nursing degree from an accredited college or university.
  • Must possess a current, unencumbered, active license to practice as an LPN in this state.
  • Must maintain and have an active CPR/BLS during employment.
  • Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
  • Experience in electronic medical records documentation.
