Job summary

Trinity Health in Michigan is seeking a licensed practical nurse (LPN) for full-time home care. The role focuses on one-to-one patient care, leveraging technology to monitor and document progress.

Expect autonomous work in a supportive, multidisciplinary team with physicians, rehab staff and home health aides. Ideal candidates will have at least five years in acute care, a Michigan LPN license, and experience with phlebotomy and wound care.

Qualifications

• Graduate of an accredited or state approved school of practical nursing and Michigan LPN licensure.
• Minimum of five (5) years experience in an acute care setting; home care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.

Responsibilities

• Provide one-to-one patient care in a home setting.
• Monitor patients and coordinate with physicians, rehab team members, social workers, and home health aides.
• Utilize EMR software and technology to document and track care.
• Maintain autonomous, organized, and compassionate care delivery in a patient's home.

Minimum Qualifications:
