The role emphasizes patient safety, accurate documentation, and collaboration with an interdisciplinary team. Training, flexible scheduling, and waived fees for continuing education are provided.

Job DetailsJob Location: Home Health Corporate Clinic Parkway - Jacksonville, FL 32216Position Type: Part TimeJob Category: Licensed Practical Nurse (LPN)Position Summary: A Licensed Practical nurse performs nursing functions under the direction of a registered nurse, follows the plans of care, participates in care planning, and performs nursing actions in accord with company policy and state law.

Job Responsibilities:

• Documents observations and care given in accord with company process according to company policies and procedures. Administers medications.
• Administers basic nursing care measure to the rehab patient and assists in giving care to the acutely ill rehab patient relative to patient age, developmental needs, and understanding.
• Admits, transfers, and discharges patients. Observes, records, and reports subjective patient symptoms.
• Assists the RN in giving physical and emotional support to the patient.
• Provides for dietary and fluid needs of the patient such as nourishment, feeding, tube feeding, observing and recording IV fluids and maintaining an accurate fluid intake and output.
• Takes and record vital signs: temperature, pulse, respirations, and blood pressure.
• Adheres to policies and procedures specific to patient rights.
• Maintains a clean and safe environment; identifies and reports hazards.
• Meets all requirements for mandatory in-services.
• Adheres to Brooks Rehabilitation and Home Health Division policies/procedures specific to universal precautions when delivering patient care.
• Participates in On Call Rotation
