Job summary

CarePartners Health Services in Asheville, NC, is seeking a Licensed Practical Nurse (LPN/LVN) to join our Home Health team. You will deliver compassionate, patient-centered care in patients' homes, collaborate with RNs and care coordinators, and support independence through education and skilled nursing.

This role requires valid LPN/LVN license, BCLS, a driver license, and an Associate degree or vocational training.

Qualifications

• BCLS must be valid at start date or within 15 days of employment.
• Licensed Practical or Vocational Nurse (LPN/LVN) required.
• Valid driver's license required to travel to patient homes.
• Associate degree or vocational school training completed.

Responsibilities

• Provide nursing care in the home, including wound care and medication administration.
• Document assessments, interventions and outcomes clearly for safe care.
• Monitor patient status and update the care team as needed.
• Educate patients and families to support medication adherence and independence.
• Collaborate with RNs, providers and care coordinators for high-quality care.

Job Summary and Qualifications

As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote their independence – caring for patients like family, one home visit at a time.

Your responsibilities will include:

• Providing nursing care in the home, including wound care, medication administration, and support during procedures
• Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
• Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses
• Educating patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
• Collaborating closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 15 days of employment start date
• (DL) Driver License
• (LPN/LVN) Licensed Practical or Vocational Nurse
• Associate Degree, or Vocational School Graduate
