Job summary

CarePartners Home Health is seeking a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) to join our Home Health Team in western North Carolina. You will provide compassionate, patient-centered nursing care in patients' homes under RN supervision.

Key responsibilities include wound care, medication administration, assessments, documentation, and collaboration with RNs and care coordinators to promote independence and safe outcomes.

Qualifications

• BCLS must be obtained within 15 days of employment start date.
• Driver License required.
• LPN/LVN license required.
• Associate Degree or Vocational School Graduate.

Responsibilities

• Provide nursing care in the home, including wound care, medication administration, and support during procedures.
• Document assessments, interventions, and outcomes clearly to support safe, continuous care.
• Monitor patient status and alert the care team of changes for timely responses.
• Educate patients and families to support medication adherence and understanding of health conditions.
• Collaborate with RNs, providers, therapists, and care coordinators to deliver high-quality home-based care.

Job Summary and Qualifications

As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote their independence – caring for patients like family, one home visit at a time.

Your responsibilities will include:

• Providing nursing care in the home, including wound care, medication administration, and support during procedures
• Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
• Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses
• Educating patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
• Collaborating closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 15 days of employment start date
• (DL) Driver License
• (LPN/LVN) Licensed Practical or Vocational Nurse
• Associate Degree, or Vocational School Graduate
