Job description

BAYADA Home Health Care is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in our Rutland Office. This office services our adult clients on a per visit basis in homes throughout Windsor and Windham County. One year of prior clinical experience as a licensed Practical Nurse is required.

As a home care nurse, you will be an integral member of a multi-disciplinary healthcare team that provides skilled nursing and rehabilitative care to clients, enabling them to receive the necessary medical care to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients in designated areas.
• Administering medication, wound care, treatments, and procedures.
• Monitoring clients' conditions and reporting changes to management.
• Following and documenting doctors' orders accurately.
• Performing client assessments as needed.
• Case management and coordination.
• Documenting observations, interventions, and evaluations using electronic medical records on a touchpad tablet.

Qualifications:

• Current Vermont license as a Licensed Practical Nurse.
• At least one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Basic computer skills; EMR experience preferred.
• Ability to travel as needed.
