Job summary

Hartford HealthCare at Home invites applications for a Visiting Licensed Practical Nurse (LPN). This role focuses on in-home patient care, coordinating with an interdisciplinary team, educating patients, and promoting self-advocacy in a supportive, technology-enabled environment.

The ideal candidate holds a CT LPN license with at least one year in LPN practice, preferably in homecare, and possesses a valid driver’s license and computer proficiency.

Qualifications

• LPN license active in CT; valid for practice.
• Minimum one year LPN experience; homecare experience preferred.
• Valid driver’s license and reliable transportation required.
• Proficiency with computer systems and EMR documentation.

Responsibilities

• Facilitate communication between patient and interdisciplinary team.
• Develop therapeutic relationships and educate to promote self-advocacy.
• Document skilled services using state-of-the-art technology.
