Job summary

Hartford HealthCare at Home in Connecticut is seeking a Visiting Licensed Practical Nurse (LPN) to join our patient-centered homecare team. You will provide skilled nursing services in patients’ homes, coordinate with an interdisciplinary team, and educate patients and families to support independence.

The role requires an active CT LPN license, vaild driver's license, and at least one year of LPN experience in homecare or related settings.

Qualifications

• Licensed Practical Nurse with valid CT LPN license.
• One year of LPN experience; Homecare experience preferred.
• Valid driver's license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to document skilled services provided.
