Locations:

• Team Green: Conducting patient home visits in Mount Dora, Eustis, and Umatilla
• Team Yellow: Conducting patient home visits in Clermont, Leesburg, Tavares, and The Villages

Role Overview:

Administer nursing care to patients per physician's orders under supervision of a Registered Nurse. Provide individualized, intermittent nursing care, teach and supervise patients and caregivers, and ensure continuity of care. Perform technical nursing skills proficiently, promote team collaboration, and document care according to policies and regulations. Complete home visits as scheduled. Continued employment depends on job performance.

Team Contributions:

• Follow all policies, procedures, and standards of care
• Provide direct patient care efficiently under RN supervision
• Perform treatments and administer medications within scope of training
• Collect patient data and contribute to care planning
• Report observations, implement physician-directed changes, and communicate updates
• Verify and implement physician orders co-signed by RN
• Communicate effectively with team members, patients, and families
• Maintain accurate clinical records
• Practice cost-effectively
