You'll provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you'll develop and manage care plans that support recovery and help patients get back to the life they love.

As a Home Health LPN, you will:

• Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, applicable licensure/certification requirements, healthcare standards, laws, and policies.
• Provide skilled nursing interventions for illness, rehabilitative needs, and preventative care, using a holistic approach in line with the plan of care.
• Apply clinical knowledge and skills to facilitate problem resolution and achieve patient outcomes.
• Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate orders from physicians as permitted by law.
• Implement the nursing plan of care using a goal/outcome-driven approach.
• Maintain contact with patients, physicians, and the healthcare team regarding needs and status changes, and participate in care coordination and discharge planning.
• Maintain confidentiality and share information only when appropriate and in the best interests of the patient.
• Ensure availability of supplies and equipment needed for safe and effective care.
• Prepare and submit documentation as required, including visit/shift notes.

Required Experience/Skills:

• Graduate of an accredited LPN program or vocational nursing school.
• Current nursing license in practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of clinical experience as an LPN/LVN, preferably in home health or hospice.

Pay Range:

• $28.00 - $40.00 per visit/unit
• $44,600 - $61,400 per year (base pay)

Scheduled Weekly Hours: 40

The pay range is an estimate for full-time employment at posting and may vary based on location and experience.
