AdventHealth in Florida is seeking a Licensed Practical Nurse (LPN) for full-time home health visits across South and West Volusia County. You will administer medications, perform assessments, and educate patients under RN supervision.

The role requires a valid LPN license, BLS certification, and 1+ year of clinical experience. Flexible scheduling, mileage reimbursement, and provided devices support the position.

Formación

Job Description

Schedule: Full Time

Shift: Monday - Friday 8am – 5pm, flexible. Every 3rd weekend. No Call!

Location 1: Home Visits covering South Volusia County. Oak Hill, South Daytona, Daytona, Daytona Beach Shores, Ponce Inlet, Port Orange, NSB, Edgewater

Location 2: Home Visits covering West Volusia County.

Mileage reimbursement provided: $0.76 per mile

Flexible scheduling with Full Time

Other duties as assigned. Develops professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations. Participates in the review process and the agency performance improvement program. Provides patient education, medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician. Displays flexibility in accepting assignment changes and participates in weekend/after-hours coverage and call rotation. Reports pertinent observations to the attending physician and RN, and implements changes in the patient care plan as directed. Communicates changes to the appropriate members of the home health care team. Demonstrates effective communication skills with all team members, patients, and families. Maintains clinical record information in accordance with established standards and within established time frames. Provides direct patient care responsibilities for assigned patients in an efficient, organized manner under the direction of the RN. Performs treatments within their scope of training and administers medications to assigned patients. Assesses patients during clinical check-in, documents pertinent medical information and vital signs, and assesses for signs of abuse or neglect using age-specific guidelines. Administers treatments, medications, and follow-up assessments ordered by the provider, verifying as needed, and responds appropriately in clinical emergencies and triage situations. Performs diagnostic and occupational medicine testing, including EKG, spirometry, audiometry, titmus, and pulse oximetry, and complete forensic testing collections in accordance with established guidelines. Collects data specific to the age of the patient and contributes to their plan of care. Receives, verifies, and implements physician orders, ensuring orders taken by the LPN are co-signed by an RN.

• Basic computer skills required including but not limited to MS Office; additional computer knowledge [Preferred]
• Venipuncture skills
• Must be proficient in the English language; additional languages [Preferred]
• Background check meets organizational guidelines
• Requires individuals to be able to make emergency and mature independent judgments and decisions
• Must have excellent communication skills, be flexible, and able to interact positively with associates
• Knowledge with a Point of Care software system for patient care documentation [Preferred]

Education

• High School Grad or Equiv [Required]
• Technical/Vocational School graduate of an accredited school of licensed practical nursing [Required]

Field of Study

• Nursing

Work Experience

• 1+ year of medical/surgical experience [Preferred]

Pay Range: $21.77 - $40.48
