Job summary

Elite Home Health Group INC in Ocala, FL is hiring a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under RN direction. This PRN role covers Ocala and surrounding areas and requires Florida LPN license, CPR, and reliable transportation.

You will administer medications, perform wound care, monitor patient status, and document care in the EMR while following physician orders and Medicare/state regulations.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Florida LPN license in good standing.
• Minimum 1 year of nursing experience preferred.
• Home health, hospice, or acute care experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to meet all required background and compliance requirements.

Responsibilities

• Provide skilled nursing care to patients in the home setting.
• Administer medications and treatments within LPN scope of practice.
• Perform wound care, dressing changes, catheter care, and other skilled nursing tasks.
• Monitor patient condition and report changes to the supervising RN.
• Reinforce patient and caregiver education.
• Follow established plans of care and physician orders.
• Complete accurate and timely documentation in the EMR.
• Communicate with RNs, physicians, and other members of the care team.
• Participate in required training, meetings, and in-services.
• Provide care in accordance with Medicare requirements, state regulations, and agency policies.
