As a Home Health LPN, You Will

• Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirement, healthcare standards, laws, regulations, and CenterWell Home Health policies and procedures.
• Provide skilled nursing interventions for patients' illness, rehabilitative needs, and preventative care, using a holistic approach in line with the plan of care.
• Apply knowledge and skills according to accepted clinical standards to resolve problems and meet patient goals.
• Consult with the RN Supervisor regarding changes in the Plan of Treatment, accept verbal orders from physicians as permitted by law, and communicate these to the RN Supervisor.
• Implement the nursing plan of care using a systematic, goal/outcome-driven approach.
• Maintain communication with patients, physicians, and the healthcare team regarding patient needs and status changes, and participate in care coordination and discharge planning.
• Maintain confidentiality and share information only when appropriate and in the best interest of the patient.
• Ensure availability of supplies and equipment needed for safe and effective care.
• Document visits, treatments, and notes accurately and timely.

Required Experience/Skills

• Graduate of an accredited LPN program or vocational nursing school.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.

Pay Range

• $28.00 - $40.00 per visit/unit
• $44,600 - $61,400 annually based on full-time employment and location.

Scheduled Weekly Hours

40 hours per week
