Licensed Practical Nurse (LPN) — In-Home Case Management

Provide compassionate, skilled in-home nursing care for clients with complex medical needs, including former energy workers and their families.

Key Responsibilities

• Conduct nursing assessments and monitor chronic conditions
• Deliver wound care and other skilled nursing interventions
• Administer medications and follow prescribed care orders
• Collaborate with RNs, physicians, and caregivers to implement individualized care plans
• Educate clients and families on care needs, health maintenance, and treatment support
• Document all care and clinical observations accurately and promptly
