Job summary

Alpha Home Health and Hospice is seeking a Licensed Practical Nurse (LPN) to deliver in-home nursing care to elderly, chronically ill, and disabled clients. You will perform essential tasks such as vital signs, medication administration, wound care, and ongoing home-based assessments.

You will document visits, communicate updates with RNs, physicians, and families, and collaborate with clinical teams to ensure high-quality, patient-centered care in home settings.
