Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in the patient's home.

• Provides services within the scope of practice as defined by state laws, according to the plan of care, and in coordination with other healthcare team members. May not alter the plan of care.
• Completes a progress note for each visit and submits notes daily.
• Observes and reports the patient's nursing needs. Instructs the patient's family in care and maintaining a healthy environment.
• Records and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician.

Education and Experience

License Requirements

• Current LPN licensure in the state of practice
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• CPR Certification
