Job summary

Delavan Health Services is seeking a Licensed Practical Nurse (LPN) for 1st and 2nd shifts with 8- or 12-hour shifts. Wages up to $38.00/hr, plus an additional $5/hour on weekends.

We are part of the North Shore Healthcare family, a leading post-acute provider in the Upper Midwest with 70 centers across multiple states. The LPN will work under the RN Unit Manager or DON, provide nursing care, coordinate and evaluate plans of care, and ensure resident safety.

Qualifications

• Licensed in the state as an LPN.
• Must have and maintain current CPR certification.
• Able to read, write, speak and understand English; basic computer knowledge; aptitude to learn company applications.
• Able to calculate drug dosage.

Under the supervision of the RN Unit Manager or DON, a Licensed PracticalNurse/Licensed Vocational Nurse performs as a licensed care giver and assumes responsibility andaccountability for assigned residents/patients for a shift of duty. Nursing care is provided throughcoordination, implementation, and evaluation of the resident’s/patient’s plan of care. The LPN/LVNadheres to the standards of care for the area, manages the environment to maintain resident/patient safety,and supervises the resident/patient care activity performance by nursing assistants. Follows all Centerpolicies and procedures. Performs duties as defined by the State Nurse Practice Act.

Required Education and Experience

• Licensed in the State as an LPN
• Must have and maintain current CPR certification
• Able to use necessary supplies and equipment (feeding pumps, oxygen, treatment trays,medication cart, etc.)
• Able to calculate drug dosage
• Meets all health requirements imposed by law
• Ability to relate positively, effectively, and appropriately with residents, families, communitymembers, volunteers and other center staff
• Able to read, write, speak and understand English. Demonstrates basic computer knowledge andability with an aptitude to learn company applications
