Job summary
Catholic Charities Family and Community Services (CCFCS) in New York is seeking a Float LPN to provide medication management across residential settings under nursing supervision. You will document medications in EMAR and support client wellness within the residence.
The role requires an NYS LPN license, a high school diploma or GED, and 2–3 years of nursing experience in MH/chemical dependency. Bilingual Spanish is preferred.

Responsibilities
- Provides medication management within the residential setting.
- Oversees residential self-medication per prescriptions.
- Documents medication self-administration in EMAR for each client.
- Procures and logs medications for each client.
- Monitors client medication adherence and communicates with prescribers and medical staff.
- Communicates with residential staff and takes part in patient discharge planning.
- Supports client wellness and coordinates with medical teams to ensure appropriate care.

Employment Type: Full-Time
Schedule: Monday thru Friday, 7am to 3:30pm
Salary: $25.00 to $30/hr

Minimum of a High School Diploma or GED and accredited LPN diploma required.
Valid NYS Licensed Practical Nurse required
Two to three years of experience related to nursing services to individuals with mental health, and chemical dependency / substance use disorder related services preferred. Bi-lingual in Spanish preferred.
Float LPN provides flexible staffing across various program settings, ensuring high-quality patient care and adapting to diverse clinical environments. Under Nursing supervision, provides medication management and offers general health and wellness support. Works collaboratively with other providers in the residence to facilitate client wellness. Maintains confidentiality and ethical standards of relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.
- Provides medication management within the residential setting
- Provides oversight during residential self-medication process to clients in the residential environment in accordance with authorized prescriptions
- Documents the medication self-administration process in each client’s electronic Medication Administration Record (EMAR)
- Procures and logs in medications received for each client within the facility
- Tracks client compliance with required medications, identifying any concerns to other Prescribers and other medical staff
- Reports medication compliance to the clinical team
- Communicates verbally with NSM and residential staff when calling off for unplanned absences.
- Provides Day to Day support of client wellness within the setting
- Observes and identifies potential client medical or medication concerns
- Facilities identification of medical concerns to appropriate medical or psychiatric staff or prescribers
- Assures identified medical concerns are addressed
- Communicates and collaborates with appropriate Agency staff, medical care providers, and related community organizations to assure coordination of client care and services.
- Completes required paperwork related to communication or services provided
- Coordinates with appropriate residential staff to facilitate transportation to necessary external medical appointments
- Promotes health and wellness within the residential facility
- Supports the development of client wellness education and activities
- Assists Medical Staff and Prescribers
- Provides Support with vital signs, and other assistance to providers as needed
- Collaborates with appropriate residential staff on client discharge plans
- Participates in staff meetings and medical/clinical supervision as directed
- Participates in relevant Agency meetings and/or trainings
- Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.
- Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse
