Licensed Practical Nurse (LPN)

Join us as an LPN and help champion compassion, dignity, and person-centered support. Your care will directly enrich the lives of individuals with disabilities and empower them to thrive.

Schedule: Monday through Friday, with normal business hours, with flexibility to adjust hours earlier or later based on needs of the individuals or scheduled appointments.

Location: Faribault, MN

Wage: $27.00 to $30.00 per hour depending on experience.

Key Responsibilities

• Provide compassionate nursing care in community and residential settings.
• Deliver care according to LPN licensure, policies, and individualized service plans.
• Train, coach, and support direct care staff to ensure safe, high-quality care.
• Maintain accurate medical records and ensure regulatory compliance.
• Help staff recognize illness symptoms and provide appropriate in-home care.
• Complete required health documentation and incident reporting.
• Assist with onboarding and ongoing clinical training for staff.
• Monitor medication administration, track errors, and support corrective actions.
• Participate in scheduled on-call rotation.
• Communicate with physicians, pharmacies, and other healthcare providers.
• Provide hands-on training in medication administration, CPR, and health-related topics.
• When providing direct care, assist individuals with ADLs, community activities, household tasks, and medications.
• Maintain confidentiality and follow all HIPAA, safety, and infection control protocols.
• Maintain active LPN licensure, CPR certification, and ongoing professional development.
• Perform additional administrative or program support tasks as needed.

Job Requirements

Graduate of an accredited school of nursing; holds a current Minnesota Nursing License. This position requires a valid Driver’s License and pass a driving record check conducted by our insurance carrier unless otherwise noted.

• Must be self-motivated and exhibit superior interpersonal communication skills.
