Qualifications

• Must possess a current, valid LPN or LVN license in state of practice.
• BLS certification is required.
• Completion of a state approved IV therapy program may be required.

Responsibilities

• Observe and document patient condition, progress and response to treatment.
• Provide nursing care under supervision of RN or physician.
• Coordinate with multidisciplinary team for plan of care and discharge planning.
• Document accurately in patient records and communicate changes to the team.
• Assist with admissions, transfers and care coordination as needed.
• Ensure continuity of care and proper handoffs between shifts.
• Maintain compliance with policies, standards of care and regulatory requirements.
• Provide patient and family education and support.
