Title: Licensed Practical Nurse – East Williamsburg Men's Shelter

Position Overview:

The LPN will report to the Assistant Director of Nursing Embedded Services and Nurse Manager for Embedded Nursing, with indirect reporting to the Director of Nursing Operations (DNO). The LPN reports to the Program Director of the designated Shelter for client assignments, daily tasks, and workshop scheduling. The role involves providing educational resources and workshops on chronic health conditions, serving as a health promotion agent to improve health outcomes within transitional and permanent housing programs.

  • Manage medication system for all clients at the shelter, including:

- Receiving and reviewing medications

- Assisting with refills

- Ensuring safe storage of medications

- Providing clients with appropriate supplies and access to medications

- Monitoring client reactions for side effects and reporting to prescribers

  • Conduct nursing screening for all new clients
  • Assist in connecting psychiatric, medical, and substance abuse issues to appropriate providers
  • Participate in weekly clinical meetings with medical staff, shelter case management, & administrative staff
  • Maintain regular communication with on-site psychiatric and medical providers, case managers, and on-site operations & administrative staff
  • Coordinate with off-site psychiatric providers as needed
  • Administer medications and injections ordered by providers
  • Provide medication teaching as needed
  • Assist clients with self-administration of medications
  • Provide ongoing health education and support to clients
  • Maintain documentation in compliance with agency policies and procedures
  • Assist with tele-psychiatry operations scheduled with tele-psychiatry providers at Project Renewal
  • Support Primary Care Clinic, Residential, and Substance Use Program coverage as needed

Education Credentials/Requirements:

New York State licensed LPN

Experience:

Experience working with homeless individuals is preferred.

Preferred Skills:

Communication: Excellent oral and listening skills.

Interpersonal Skills: Ability to interface effectively with clients and all levels of staff.

Computer Skills: Knowledge of Microsoft Office, especially Word and Excel.
