Licensed Practical Nurse - East Williamsburgh

Title: Licensed Practical Nurse – East Williamsburgh Men's Shelter

Position Overview:

The LPN reports to the Assistant Director of Nursing Embedded Services and Nurse Manager for Embedded Nursing, with indirect reporting to the Director of Nursing Operations (DNO). The role involves client assignments, daily tasks, and workshop scheduling under the supervision of the Program Director of the Shelter. The LPN provides educational resources, conducts workshops on chronic health conditions, and acts as a health promotion agent to enhance health outcomes in transitional and permanent housing programs.

Responsibilities include:

  1. Managing the medication system for all shelter clients, including:
  • Receiving and reviewing medications
  • Assisting with refills
  • Ensuring safe medication storage
  • Providing clients with appropriate supplies and access to medications
  • Monitoring client reactions and reporting side effects to providers
  1. Performing nursing screenings for all new clients
  2. Connecting clients with psychiatric, medical, and substance abuse providers as needed
  3. Participating in weekly clinical meetings with medical staff, case management, and administrative personnel
  4. Maintaining communication with on-site psychiatric and medical providers, case managers, and staff
  5. Communicating with off-site providers when necessary
  6. Administering prescribed medications and injections
  7. Providing medication education and assisting clients with self-administration
  8. Offering ongoing health education and support
  9. Maintaining documentation in accordance with agency policies
  10. Assisting with tele-psychiatry operations as scheduled
  11. Supporting coverage for Primary Care Clinic, Residential, and Substance Use Programs as needed

Education and Credentials:

New York State licensed LPN

Experience:

Experience working with homeless individuals preferred.

Preferred Skills:

  • Communication: Excellent oral and listening skills required.
  • Interpersonal Skills: Ability to interface effectively with clients and staff at all levels.
  • Computer Skills: Proficiency in Microsoft Office, especially Word and Excel.
