Job summary

Fresenius Medical Care is seeking an LVN for PPS to provide direct patient care to dialysis patients in a clinical setting. You will document, triage via phone, and support the care team with accurate charting and timely information exchange.

Responsibilities include administering medications, assisting during procedures, and ensuring patient comfort and safety while adhering to state regulations and company policies.

Qualifications

• High School Diploma required.
• Must possess current LVN license.
• 2–4 years of related experience preferred.
• Excellent organizational and planning skills, time management and communication abilities.

Responsibilities

• Facilitates communication among providers, staff, patients, and partners.
• Gathers information for provider assessments and previews charts.
• Monitors patient status during procedures and documents data.
