Job summary

Collierville Health and Rehab in Tennessee is seeking qualified LPNs and RNs to join our bedside care team. You will consult with the healthcare team, administer medications, monitor vital signs, and participate in discharge planning to support residents' well-being.

Qualifications

• Current RN/LPN license in the state.
• At least one year of LPN or RN experience in a long-term care setting preferred.

Responsibilities

• Consults and coordinates with healthcare team to plan and evaluate patient care.

• Records patients' medical information and vital signs.
• Monitors equipment and interprets electronic displays.
• Participates in discharge planning.
• Provides bedside care and assists with examinations.
• Maintains timely and accurate care documentation.
