Creekside-Health- seeks a Licensed Practical Nurse (LPN) to provide direct nursing care and support daily clinical operations within our skilled nursing facility. This role ensures residents receive quality care in accordance with established standards and regulations, with a compassionate, patient-centered approach.

Responsibilities include administering medications and treatments, monitoring and reporting changes in resident condition, documenting assessments in the EMR, assisting residents

Qualificações

• Current LPN license in the state of employment.
• Graduate of an accredited practical nursing program.
• Strong communication and clinical skills.
• Ability to work both independently and collaboratively.
• Detail-oriented with effective time-management skills.
• Availability to work various shifts, including weekends and holidays.
• EMR experience preferred but not required.
