Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city.

Nurses work in collaboration with the interdisciplinary team to provide patients with care, education, and support. LPNs participate in the planning, implementation, and evaluation of nursing care, based on the objectives, standards, and policies of practical nursing. LPNs work under the direction of a Registered Nurse and/or Physician.

Responsibilities:

  1. Maintains a comfortable, orderly, safe, and clean environment for patients.
  2. Measures & records patients’ vital signs (such as height, weight, temperature, blood pressure, pulse, respirations, food/fluid input/output, etc.) in the electronic health record.
  3. Assists with bathing, dressing, and personal hygiene of the patient.
  4. Monitors catheters.
  5. Dresses wounds.
  6. Assists with patient positioning, standing, walking, and transferring.
  7. Collect samples of blood and/or urine for testing.
  8. Performs routine laboratory tests as ordered.
  9. Prepares, administers, and documents prescribed medications via the ordered route.
  10. Reports adverse reactions to medications or treatments according to the policy regarding medication administration.
  11. Assists in relevant patient teaching.
  12. Records all care information concisely, accurately, completely, promptly, in the appropriate format, and on the appropriate forms/templates.
  13. Performs other position-related duties as assigned, depending on the assignment setting.
  14. Direct care provided by unlicensed staff.
  15. Upholds HIPAA regulations.
  16. Floats among various clinical services, where qualified.
  17. Punctual and dependent on assigned/confirmed shifts.

EMERGENCY RESPONSE

  1. Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, burns, and recovering post-operative patients.
  2. Provide clinically appropriate care as directed by the Registered Nurse and/or Physician.
  3. When clinically indicated, provide care, such as CPR, AED, prevent shock, control severe bleeding, prevent spinal damage, etc.
  4. Monitor patients' condition and keep it stable while in route to the clinic.
  5. Aid in the preparation of the transfer of patients by Urgicare and/or EMS to the emergency units or hospitals.
  6. Document the medical treatment that is given to the patient.
  7. Keep the equipment clean after use; check and replace damaged and used supplies.
  8. Collaborate with the pharmacy to replenish supplies.

PHYSICAL DEMANDS

  1. Ability to lift fifty (50) pounds.
  2. Moving, lifting, or transferring patients may involve lifting up to 100 pounds.
  3. Ability to stand for extended periods.
  4. Fine motor skills.
  5. Visual acuity.

Schedule: Sunday, Monday, Wednesday, Thursday, Saturday: 4pm-12am

Scheduling note: Nurses are required to work every other weekend.

This position is unionized through 1199 and granted 1199SEIU benefits.

Qualifications:

  1. Graduate of an accredited LPN program.
  2. Licensed Practical Nurse currently registered to practice in New York State.
  3. Demonstrated clinical competence and skills in problem-solving, communication, priority-setting, and interpersonal relationships, with experience in clinical nursing practice as a medical/surgical, behavioral health, or correctional health nursing experience preferred.
  4. Demonstrated commitment to continuing professional development through attendance at continuing education in-service sessions and participation in professional/organizational committees/councils.
  5. Basic Life Support Certification is required; ACLS is encouraged.
