Job summary

Concordia Visiting Nurses in Canonsburg, PA seeks an experienced Licensed Practical Nurse (LPN) to join our home health team. You will provide skilled nursing care, monitor vitals, and educate patients and families in a supportive, compassionate setting.

Ideal candidates have at least 1 year of LPN experience, strong communication, and the ability to work independently within a collaborative team. Flexible scheduling and comprehensive benefits are offered.

What you’ll do as a Licensed Practical Nurse LPN:

As a Home Health LPN, a vital part of the patient's care team is provided by the employee, offering skilled nursing care and support. This includes tasks such as:

• Monitoring vital signs
• Assisting with wound care and personal care
• Educating patients and families
• Maintaining medical records
• Collaborating with other healthcare professionals. Also, provide emotional support and companionship, building genuine connections with patients.
