Five Star Nursing is seeking an licensed practical nurse (LPN) in New York to provide direct patient care in a long-term care setting. The role requires current licensure, CPR certification, and a focus on accurate history taking and vital signs monitoring. Training and weekend schedules may be available.

We value compassion, reliability, and attention to detail. Experience with SigmaCare is a plus, and new grads are welcome to apply for full-time or part-time shifts.

Qualifications

• Current LPN license and registration.
• CPR certification.
• Malpractice insurance.
• Resume required.
• Physical examination.
• MMR & Varicella titers.
• PPD or chest X-ray if positive.
• 1 personal and 1 professional reference letter.
• Original Social Security Card.
• Original non-expired state ID.
• Proof of citizenship/immigration documents.
• SigmaCare experience is a plus.
• Long-term care experience welcome; new grads considered.

Responsabilités

• Record a patient’s medical history accurately.
• Take and record measurements (BP, temp, HR).
• Observe patients under treatment for progress and side-effects.
• Monitor patient condition and chart fluid intake/output.
• Assist with daily activities (eating, bathing).
• Administer injections and prescribed medications; wound care.
• Collect samples for lab testing as assigned.
• Provide emotional support and communicate with families.

Requirements of the (LPN) Licensed Practical Nurse

• Current LPN license and registration
• CPR certification
• Malpractice insurance
• Resume

• Original Social Security Card
• Original Non expired State Identification

• Sigma Care experience (computer system, patient information) a plus
• Long-term care experience a big plus, but new grads welcome as well!

Responsibilities for the (LPN) Licensed Practical Nurse

• Record a patient’s medical history accurately
• Take and record measurements of blood pressure, temperature, heart rate etc.
• Observe patients under treatment to identify progress, side-effects of medications etc.
• Monitor patients’ condition including fluid intake and output and compose patient charts
• Assist ailing patients in daily necessary activities such as eating, bathing etc.
• Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
• Collect samples for lab testing as assigned
• Provide emotional and psychological support when needed
• Communicate with patients’ family or friends to provide advice, comfort and release instructions
