Ocean Meadow Senior Living in Clinton, CT is seeking a Licensed Practical Nurse (LPN) for a 7AM–3PM shift. The role includes medication administration, admissions support, care plan implementation, and clinical tracking under direction.

Requires current LPN license, CPR certification, and 1–2 years of nursing experience, preferably with elderly residents. In-person, full-time with option for part-time or per diem, compensation $32–$34/hour.

Qualifications

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program (per state requirements).
• Minimum of 1 to 2 years of experience working as a nurse preferred.
• Previous experience working with the elderly in a paid or volunteer position is highly desirable.

Responsibilities

• Medication administration for residents in compliance with Federal & State regulations.
• Assist with admissions of new residents, including assessments and documentation.
• Assist with implementing acute and long-term care plans or service plans and data collection.
• Maintain clinical system tracking (wounds, infection control) under direction.
• Provide general health guidance and consultation within the community.
• Notify physicians and/or family members of health changes with proper documentation.
• Respond quickly to emergencies and coordinate care.
• Maintain communication with departments and staff.
• Provide educational meetings for residents and staff.
• Understand and apply Safety & Disaster Plan.
• Maintain confidentiality of residents’ information.
• Attend required training, in-service, and staff meetings.
• Maintain a safe working environment and professional demeanor.
• Perform other duties as assigned.

Essential Functions:

• Responsible for medication administration for the residents in compliance with Federal & State regulations
• Assist with admissions of new residents, including assessments and documentation
• Assist with the process of implementing acute and long-term care plans or service plans an/or the collection of data
• Responsible for clinical system tracking (i.e., wounds or infection control) under the director's direction
• Provide general consultation regarding health-related concerns within the community
• Notify physicians and/or family members of any change in the resident’s health and provide proper documentation

• Maintain close communication with all departments and staff members
• Provide educational meetings for residents and staff
• Understand the role of the Safety & Disaster Plan

• Attend all required training, in-service, and staff meetings
• Maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices
